Investment Instruction Form This form should be completed by all FIPS members to confirm how you would like your future pension contributions to be allocated between the different funds and/or change where existing pension savings are invested.
AVC Form To indicate the frequency and amount of AVCs you wish to pay.
AVC Form NON FIG To indicate the frequency and amount of AVCs you wish to pay.
Expression of Wish Form To assist the Pensions Board in making the selection as to who should receive these death benefits and in what proportions in the event of your death.
